The Hellfire Amulet is a craftable amulet available to all players once they have purchased the recipe for it from Squirt the Peddler in Act 2.
The amulet comes in 3 forms, one for each main stat, and is usable at any level. The stats on it are randomised except for your chosen main stat, but each amulet is guaranteed to have two things on it:

- Socket — Patch 2.3 gave all Hellfire Amulets a guaranteed socket roll.
- Passive Skill — Every Hellfire Amulet will have a random passive skill from your class, meaning that when you wear it, you will essentially be able to have 5 passive skills for your chosen build.
To craft a Hellfire Amulet, you will first need to obtain the recipe to do so. The recipe can be purchased from Squirt the Peddler, who can be found in the Hidden Camp in Act 2, and costs 5,000,000 Gold. Take the recipe to the Jeweler to learn it.
Each amulet will require the following materials:
- 1 Leoric’s Regret
- 1 Vial of Putridness
- 1 Idol of Terror
- 1 Heart of Fright
- 10 Forgotten Souls
The Forgotten Souls are fairly easy to obtain, simply coming from salvaging legendary items, but the other 4 ingredients come from the Infernal Machines.
The Infernal Machines are dropped by the Keywardens of the first 4 acts of the game, with each one dropping a specific machine, which grants a specific material when you complete the mini-rift that is opened by using the machine.
- Act 1 — Odeg, the Keywarden of Regret drops the Infernal Machine of Regret and is found in the Fields of Misery. This will reward you with Leoric’s Regret upon completion of its event.
- Act 2 — Sokahr, the Keywarden of Putridness drops the Infernal Machine of Putridness and is found in Dahlgur Oasis. This will reward you with Vial of Putridness upon completion of its event.
- Act 3 — Xah’Rith, the Keywarden of Terror drops the Infernal Machine of Terror and is found in Stonefort. This will reward you with Idol of Terror upon completion of its event.
- Act 4 — Nekarat, the Keywarden of Fright drops the Infernal Machine of Fright and is found in the second tier of the Gardens of Hope. This will reward you with Heart of Fright upon completion of its event.
The number of machines that drop from the Keywardens is not fixed and can vary depending on the difficulty you kill them on.
Below is a list you can refer to that outlines the drop rates of the machines by Torment level.
- Normal — Master: the Keywardens do not spawn on these difficulties.
- Torment 1: 50% chance of 1 machine.
- Torment 2: 75% chance of 1 machine.
- Torment 3: 90% chance of 1 machine.
- Torment 4: 100% chance of 1 machine.
- Torment 5: 100% chance of 1 machine and 10% chance of 2 machines.
- Torment 6: 100% chance of 1 machine and 20% chance of 2 machines.
- Torment 7: 100% chance of 1 machine and 30% chance of 2 machines.
- Torment 8: 100% chance of 1 machine and 40% chance of 2 machines.
- Torment 9: 100% chance of 1 machine and 50% chance of 2 machines.
- Torment 10: 100% chance of 1 machine and 60% chance of 2 machines.
- Torment 11: 100% chance of 1 machine and 70% chance of 2 machines.
- Torment 12: 100% chance of 1 machine and 80% chance of 2 machines.
- Torment 13: 100% chance of 1 machine and 90% chance of 2 machines.
- Torment 14: 100% chance of 2 machines and 25% chance of 3 machines.
- Torment 15: 100% chance of 2 machines and 75% chance of 3 machines.
- Torment 16: 100% chance of 3 machines and 25% chance of 4 machines.
After collecting your Infernal Machines, you can use them to obtain the final step in the crafting collection process.

In New Tristram in Act 1, head towards Brother Malachi the Healer, who is marked by a heart on your minimap. You can find him near the Mystic.
He will be standing next to a house that has a boarded-up door. Use any spell to break the door and enter the Heretic’s Abode. Here, you can use your Infernal Machines to open portals to the different Infernal realms.
Each realm has 2 bosses inside that correspond with the theme of their realm, except for the realm of Fright, which features only 1 boss, but he can summon others to aid him.
The fights themselves used to be quite challenging, but with the constant buffs to player power, these will be extremely easy to complete once you have made a small amount of progress into being Level 70 and at least have a full set of gear.
The amount of each material that drops will depend on the difficulty you defeat the bosses on, as shown below, with a maximum of 4 of each item on Torment 16. Note that you cannot open any Infernal realms on a difficulty lower than Torment 1.

The Realm of Regret is the first realm and contains a boss from Act 1 (the Skeleton King) and Act 2 (Maghda) and will be fought in the same setting as the Skeleton King fight from the campaign and bounty board.
The only real mechanic that you may need to pay attention to is Maghda’s shield, which renders her invulnerable until you kill the adds she has summoned. It is normally easiest to just burn down the Skeleton King quickly and then focus on her.
This realm will drop Leoric’s Regret.
The Realm of Putridness is the second realm and contains a boss from Act 3 (Ghom) and Act 4 (Rakanoth) and will be fought in Ghom’s boss room.

Avoid Ghom’s slimes and kill Rakanoth first to avoid his 1-shot teleport ability; note that on classes with higher toughness, this will not be as much of an issue.
This realm will drop Vial of Putridness.
The Realm of Terror is the third realm and contains a boss from Act 2 (Zoltun Kulle) and Act 3 (Siegebreaker Assault Beast).
Dodge the attacks of Zoltun Kulle, most notably his fireballs and the rocks he drops from the sky, and move away from the Siegebreaker whenever you can.
This realm will drop Idol of Terror.
The Realm of Fright is the most difficult of the realms and contains Diablo from Act 4.
Dodge his fire and lightning attacks as best you can, ensuring to not get snared. Diablo will summon a random boss upon reaching 75% and 50% remaining Health, so be prepared for their spawns.
Focus down the new boss before returning to Diablo, otherwise you may accidentally trigger the next spawn and have to deal with all 3 at the same time.
This realm will drop Heart of Fright.
The Hellfire Ring is largely obsolete and is never used in the current era of the game.
